1. Get a Business License and create a company name.

2. Get Vehicle and have registration changed to "For Hire"

3. Get Commercial Insurance

4. Get City of Seattle Business License Tax Certificate

5. Get a Limo Inspection

Needed: 6 Flairs or 3 Triangles and a Fire Extinguisher 5-B C or more secured in the trunk.

6. Send documentation and payment to Olympia

You will need the Business License Application, Limousine Carrier Chauffeur Addendum,
a copy of the vehicle registration for each vehicle, Certificate of Insurance and the
Vehicle Inspection Report from the city of Seattle. This takes 4-6 weeks to return.

Get your Medical Examiners Certificate
$100.00 - $150.00
Your physical must be conducted by a licensed medical examiner listed on the FMCSA
National Registry.

Go to Alliance 2020 get your Chauffeur Credentials

Proof of Paid PSLA Application Fee and First Years' Dues
Copy of State Business License (Address Must Match Address of Subscriber: Street
Address Required: PO Boxes are NOT Acceptable)
Copy of Local Business License
Proof of Commercial Liability Insurance (ACCORD Certificate or Equivalent)
List of For Hire Vehicles, including Make, Model, Year and License Plate Number
Limousine Certification and Proof of Commercial Automobile Insurance for Each For-Hire
Vehicle
Copy of Current Driver's License of the Individual Signing the Agreement

Provide Port of Seattle all documents received from Olympia.

10. Bring all documentation to Shuttle Express to get scheduled for
work.

Limitations of Translatable Documents
